I don't think the problem started immediately after changing the weapon, but not 100% sure.Upgrade your followers' gear and skills whenever you get the chance. Changed her weapon recently though, to Azurewraith, but nothing else. I haven't changed her follower item or armor in weeks. No idea what caused this, but it seems like at least two of her equipment pieces were not functioning. Went back into another GR and no longer was taking damage, problem solved. I clicked on one of the greyed-out skills and that caused all 8 skills to become activated. Then I noticed that even though she had the focus that grants her access to all skills, half her skills were greyed out as if she didn't have that focus at all. Double-checked to make sure she still had the Spaulders and she did. Last night I was pushing GRs, dying some, and when I finished and returned to town, I noticed my gear was taking damage. I also run the Enchantress focus that gives her access to all skills (with the two damage reduction legendary gems). I run GR pushing with my Enchantress with the Spaulders that Emanate gear damage prevention.
